FOE FACTS! is an importantitem in OMORI. It is a journal given by HERO to OMORI during the PROLOGUE. It can be opened by the player via the IMPORTANT tab of the menu. It basically lists most of the enemies fought throughout the party's adventures in HEADSPACE.
HERO gives OMORI the journal after they defeat the LOST SPROUT MOLE wrecking BASIL's flowers.
LIST OF FOES
After meeting or defeating a foe, OMORI and his friends would write down their observations that served as the foe's descriptions. OMORI's descriptions are mostly long and well-detailed, showcasing his talent for writing. The rest of the group's descriptions, however, are mostly short funny phrases.
The list below depicts all enemies necessary to complete the journal. There are none of the FARAWAY TOWN enemies nor the SOMETHING or BLACK SPACE related enemies as the journal simply shows the many creatures encountered in HEADSPACE. LIFE JAM GUY, SNALEY, and BOSSMAN HERO are also not included in the entries. This even extends to the simulations of AUBREY, KEL, HERO, and MARI from the BOSS RUSH.

COMPLETION
In order to obtain the "FOES FILED!" achievement, the FOE FACTS book must be completely filled out. This is only possible in the OMORI ROUTE, as certain enemies and areas are only unlocked during it. Most enemies can be found through natural progression, but there are some that the player should take special care to find or encounter if they are attempting to complete the book.
There are a number of enemies that are missable, and forgetting to fight them would completely ruin the completion of the book:
PLUTO (referred to as ??? in-battle) must be encountered behind a tree in OTHERWORLD before he's gone and cannot be battled again, as PLUTO (EXPANDED) is considered a different foe. He will stay there until the chase scene with ROSA.
KING CARNIVORE is a foe that can be missed by dropping the anvil on its head, defeating it without starting the encounter. It is located in SPROUT MOLE VILLAGE, below MAYOR MOLE's room.
GHOST BUNNIES must be encountered while in the LOST FOREST, as the forest becomes permanently inaccessible after leaving.
HAROLD, MARSHA, THERESE, LUCIUS, HORSE HEAD, and HORSE BUTT are also missable if they are not encountered before accepting KEEPER OF THE CASTLE's offer during ONE DAY LEFT.
The following enemies are easy to miss, but won't lock you out of the completion:
DUST BUNNIES are found in two areas or one depending on how far you've gone through the game; one within a bookcase in BASIL'S HOUSE shortly after he's gone missing, and another within a bookcase in SMOL'S HOUSE.
BIG STRONG TREE looks exactly like many of the trees surrounding the area outside of the TRAIN STATION, but can be found by running from a fight and looking for the flashing sprite. It's worth noting that this tree has a slight difference in appearance to the other trees via multiple discolored pixels.
THE EARTH can be intentionally encountered in the area behind SPACE BOYFRIEND'S house. This, however, is not a necessity, as she can be fought again later alongside PLUTO (EXPANDED) during the last day of the OMORI ROUTE.
SPROUT MOLE? can be confusing as it has no overworld sprite, and rather spawns on a random chance when encountering a regular SPROUT MOLE in PYREFLY FOREST.
UNBREAD TWINS and the other grain-related enemies are found in BREAVEN, a secret area in ORANGE OASIS.
MUTANTHEART and PERFECTHEART require the player to return to HUMPHREY during the last day of the OMORI ROUTE. ROBOHEART is in the last room of the hard puzzle, and should be accessible right after she flies away.
SPACE EX-HUSBAND is found in SNOWGLOBE MOUNTAIN on the last day of the OMORI ROUTE.
TRIVIA
In FARAWAY TOWN, there is a poorly drawn journal named "FOE FACTX!" made by ANGEL, which can be seen in his room. When checking the booklet, it depicts the foes ANGEL has "fought" before. They consist of ARTIST, KEL, and SUNNY.
It's also possible the HEADSPACE variant takes inspiration from ANGEL's FOE FACTX! booklet.
Some enemies, such as DUST BUNNY, do not have a visible sprite in the overworld. Despite this, they are still given a unique sprite in their respective FOE FACTS! entry.
The BUN BUNNY from DINO'S DIG before ONE DAY LEFT will not be registered unless it is defeated. This is likely an oversight left by the developers.
